Please ask for a new patient pack at Reception. Once we have received your completed forms you will
be asked to attend a new patient health check with the Practice Nurse.
New Patient Health Check
This is an extremely valuable check up as it can identify potential problems and give base line
readings for future reference. It introduces you to the practice and the services we can offer you and your family.
