General Information

Change of Address/Personal Details

If you change your name, address or telephone number please notify us as soon as possible so we can maintain accurate records. If you move outside of our designated practice area you will be asked to transfer to the list of a doctor nearer to your new home.

Test Results

Results of investigations undertaken in the practice, e.g. blood tests, smears, X-rays are available from the receptionists. Please allow 7-10 days for the results to come through and telephone after 11.00am. If the doctor feels it is urgent that you be seen in surgery as a result of a test we will contact you.

Sickness Certificates

NHS certificates are only required after the first week of an illness. You can complete a self certificate for the first week. Self certificates can be obtained from your employer or from the surgery. There is a fee payable for all private certificates.

Private Medicals & Other Forms

Insurance, employment, driving medicals, etc. are conducted by appointment. Many forms that need to be completed by a Doctor, or require their signature, may incur a fee. Please contact reception who will advise you on the relevant forms and current fees.

Temporary Residents

We are pleased to see visitors to the area as temporary residents. Please contact the reception staff to register and make an appointment.

Smoking Policy

Yetminster Health Centre is totally committed to adhering to a no smoking policy in all of its buildings and on all property. We offer a smoking cessation advice service within the practice.

Car Parking

As you are aware we have a small car park at the surgery and the entrance has to be used for all pedestrians accessing the main entrance. This area is for staff only plus one Disabled parking space.

We would appreciate your co-operation in this so as to make it a safer area for all concerned but especially for those who may have mobility restrictions. There are two wheelchairs available for those patients with mobility difficulties, in getting from their transport into the surgery. If you have any queries about the use of these wheelchairs please ask a member of staff at the practice.

Referrals to Secondary Care

At times it may be necessary for you to be referred to another doctor or a hospital. The doctor here will be able to use the Choice & Booking system and you will be given a letter with the information you need to make your appointment with the secondary care clinicians at a time you find convenient to. You may have a copy of the referral letter if you so wish.

Yetminster Medical Fund

Any donations that are made to the Health Centre are put into the Medical Fund and contribute towards new equipment for the benefit of the practice.
